I came across a question of a past rated contest and there is a major testcase missing in it. My friend submitted a solution for it and it was accepted but it had a major flaw in it. Still his solution was accepted. So, is there any way to hack that solution or any way to add the missing testcase to the pool?

As far as I know, you have to be at least CM to do that.
This is called uphacking.
I believe that you need to be Division 1 to uphack, and uphacking is only available for a week after the contest is over. You can see this blog from Mike for more details. The blog says at the end that the rating requirements will be lowered and the time period extended, but as far as I know that has not happened.
